I go to Vegas two or three times a year and always have one of my dinners here. And I always get the same thing. The chicken parmigiana. My wife gets something different and we share. Check out their reviews online. Has the feel of what might have been a mob hangout at some point in years past. old Pueblo and Brad have some great suggestions too that I will check out on our next trip .

We'll NEVER go fancy in Vegas. We're staying in Park MGM. Good location, value and only place on the strip that doesn't allow smoking. We'll hit Primrose (Park MGM) for brunch every day. We ate at an old favorite of ours last night-Nora's (Italian). After gambling in NYNY most of the afternoon in we'll get a steak there at Gallagher's. Tomorrow night it's Lotus of Siam at their new location and our last night we'll eat at Yardbird in the Venetian.
Any recommendations? We ate at Bavette's (Park MGM) during the NCCAs last March. Very good place.
Bavette's is solid. If you like Bavette's, I have some others for you'd LOVE. There's 3 gems in the downtown area alone... phenomenal. North strip has the best restaurants imo.... and I'm sure Fountain Blue will add another when it opens on Dec 13th.... Dinned at Ocean Prime (LVB/Harmon) last week,,,, Outstanding! Kinda "fancy" tho. Nothing too serious, I just throw on a jacket with a button down if it's "upscale." They all have close to the same prices. Prime at Bellaigo maybe a little more. Some solid Italian here as well. I have an amazing one for the next time you come out. Not fancy at all. The chef is a true artist. Food is next level.
Lotus is great. First time I ate there was about 20+ years ago at the old loc on Sahara, before I had a place in LV. Same food at the Flamigo Av location. Have fun!

A lot of the best restaurants are off strip. Downtown continues to impress.
If you have a car and willing to drive:
EDO- Tapas, small plates
Al Solito Posto- italian
138-steak
Ellis Island- a little bit of everything, well priced.
Kabuto-japanese
All these places are casual, upscale maybe but not fancy. It's more of a price point decision.
Pizza Rock downtown. I like the pies and it's a quick walk off fremont
